Once your first appointment is scheduled, we'll send you a link to our secure patient portal to complete a few forms ahead of time. These help us understand your background before we ever sit down together, so your first session can focus on you, not paperwork.

Coordinating Care With Another Provider
If you'd like us to stay in contact with another provider involved in your care, a psychiatrist, primary care physician, or anyone else, we're glad to. We just need your written authorization first.
📄 Exchange of Confidential Information
This isn't limited to new clients, fill it out whenever it's relevant, at intake or anytime after, and return it to our office.
